How information about you helps the NHS to provide better care
Confidential information from your medical records can be used by the NHS to improve the services offered so they can provide the best possible care for everyone. This information along with your postcode, date of birth, gender and NHS number but not your name, are sent to a secure system where it can be linked with other information such as diagnoses and medication. This allows those planning NHS Services to use information in a way that does not identify you. The data is governed by data protection laws.
YOU HAVE A CHOICE.
If you are happy for your information to be used in this way, you do not need to do anything. If you have concerns or wish to prevent this from happening, please speak with a member of staff or ask for a copy of the leaflet "How information about you helps us to provide better care". Vaccines are available between October – March. Please see the noticeboard for further information or pick up a leaflet at the surgery.

Electronic Prescription Service
We introduced an Electronic Prescription Service in December 2012. Please fill in the form, by clicking this link, to nominate the Pharmacy you wish to use. Please return your completed form as soon as possible to your Pharmacy or to us.
Travel Risk Assessment
If you are thinking of travelling abroad, you may require advice on vaccinations. To enable our Practice Nurse to compile your personal plan, please download and complete our travel risk assessment form (pdf). This should then be returned to the Surgery as soon as possible for assessment.
Please note: we need six weeks notice for travel vaccinations

If you telephone outside surgery opening times you will be asked to contact the out-of-hours service NHS 111, who will assess your clinical requirements and advise you accordingly.
Extended Hours Surgeries
The surgery holds extended hours surgeries for patients at the following times:
Tuesday & Thursday - 07.00 - 08.00am
Wednesday 18.30 - 19.30pm
We are unable to take telephone calls during the extended hours surgeries so please make your appointment during normal surgery hours. Please be aware that we are unable to offer a nursing or phlebotomy service during these surgeries.
The surgery has a number of designated disabled parking spaces for patients at the rear of the building. There is a lift available.
There is a limited number of parking spaces (first come, first served) at the rear of the building. Please help us to manage this area by only parking for the duration of your visit to the surgery.
As from 19th May 2014, Parking Eye will be operational in our car park, using an enhanced camera based system that logs your registration number when you enter and exit the car park.
To qualify for 1.5 hours FREE PARKING you will need to enter your FULL registration details into the touch screen terminal that is located on our reception desk and press enter. You will need to enter your vehicle details EVERY time you use the car park.
Failure to comply with the terms and conditions (i.e.) parking in an incorrect space/ on yellow lines/ on the hatched area/ overstaying the 1.5 hours will result in a Parking Charge of £70, which will be reduced to £40 if paid within 14 days of issue.
