Travel Risk Assessment
If you are thinking of travelling abroad, you may require advice on vaccinations. To enable our Practice Nurse to compile your personal plan, please download and complete our travel risk assessment form (Word format). This should then be returned to the Surgery as soon as possible for assessment.
Please note: we need six weeks notice for travel vaccinations
Disabled Access
The surgery has a number of designated disabled parking spaces for patients at the rear of the building.
Parking
There are a limited number of parking spaces at the rear of the building. Please help us to manage this area by only parking for the duration of your visit to the surgery.
