Travel Risk Assessment
If you are thinking of travelling abroad, you may require advice on vaccinations. To enable our Practice Nurse to compile your personal plan, please download and complete our travel risk assessment form (Word format). This should then be returned to the Surgery as soon as possible for assessment.
Access And Parking
Access to the surgery premises is via lifts or staircase.
the surgery has parking for patients at the rear of the building. There are a number of spaces designated for disabled drivers.
