How Do I.... Register With The Practice?

Prospective patients moving into our area (see map page) may wish to apply to register with the practice. To do this it is preferable to bring along your medical card, which has your NHS number on it, enabling us to quickly process your registration. If you do not have your medical card there is an alternative form for completion, just ask the receptionist. You will need to bring with you some form of photographic proof of identity, such as your passport or driving licence. We will also need to see proof of residency; this could be a recent (within three months) bank statement or utility bill.

Your new patient health questionnaire will be reviewed by one of our practice nurses who will suggest you make an appointment for a health check if she feels it is appropriate.  If you would like a new patient check at registration, please ask the receptionist to organise one for you.
